More important is an assessment of the ethical issues and how tensions between them might best be resolved (as we are attempting to do in this report). These included seeking expert input from established groups and making explicit the processes or expertise it has drawn on in reaching conclusions about social, ethical and legal issues. Several of these tests are diagnostic and remove the need for invasive testing altogether. Women and couples with a family history of a genetic condition have a number of options available to them if they wish to avoid their biological children inheriting the condition. One option is to conceive naturally and perform genetic testing on the fetus once this is possible (this varies depending on the procedure). The evaluation takes into account the seriousness and prevalence of the condition, the purpose, performance, clinical utility and price of the test and any ethical, legal and social considerations. For each test, a clinical care pathway has been developed that outlines how the test should be offered and delivered. The focus of the project was prenatal diagnosis for women who have an increased chance of having a fetus with a specific condition, rather than genetic screening of whole populations. The guidelines aim to provide a flexible framework for ethical clinical care and describe general principles, logistical considerations, clinical care and counselling topics in the context of prenatal diagnosis. This may be in advance of a pregnancy if a couple knows that any child they have will have a chance of a genetic condition. This may be because one of the parents is affected by a genetic condition, they have already had an affected child, or because they have a family history of the condition. A referral may also arise in the course of a pregnancy if problems are identified on an ultrasound scan, because of problems with maternal health, or if the couple becomes aware of a potentially relevant family history. Therefore, the two major groups of referred women and couples are those who already have a good knowledge of the condition in advance of having to make a decision, and those who are forced to make a decision very quickly under difficult circumstances. The criteria set out the circumstances in which testing should take place, such as the gestation of the pregnancy and the carrier status of the couple. Women and couples are often already well informed about the condition if they have the condition themselves or it is in their family. Healthcare professionals involved in the delivery of prenatal genetic testing services tend to be appropriately skilled, and information is given in a timely and non-directive fashion. However, we did hear some reports of women being directed towards termination and a tendency for the focus to be on medical problems associated with the condition rather than information about day-to-day life. This can help women to make informed choices about termination or to prepare for the birth of a disabled child. It is possible that, in the future, more women with a family history of a genetic condition or with anomalous ultrasound scans may choose to have prenatal diagnostic testing if there is a safe, accurate test available that can be carried out early in pregnancy. Increased uptake may lead to an increase in the number of women receiving positive results, and thus an increase in both the number of women continuing a pregnancy and the number of women terminating a pregnancy. Tumors that cannot be classified unequivocally as either completely benign or definitely malignant are frequently referred to as premalignant or precancerous lesions. Because such lesions cannot be distinguished clinically from squamous cell carcinoma, surgical excision is usually recommended. It occurs in middle-aged to older adults and is most evident in the corneal epithelium. It is frequently associated with one or more foci of conjunctival squamous cell carcinoma (see later in this chapter). It is usually managed by surgical removal of the visibly abnormal epithelial cells or topical therapy using mitomycin C, 5-fluorouracil, or interferon alpha-2b. Lymphoid Hyperplasia of Conjunctiva 259 Lymphoid hyperplasia of the conjunctiva is an infiltrative disorder of the conjunctival substantia propria in which the infiltrates are composed of mildly to moderately atypical lymphocytic cells. This lesion is generally regarded as borderline between benign inflammatory lymphoid infiltrates and malignant lymphoma (see later in this chapter). It affects both males and females, and it can be unifocal, multifocal, or diffuse in one or both eyes. If the lesion is prominent, incisional biopsy or excision is indicated to exclude malignant lymphoma. Some patients with lesions classified pathologically as lymphoid hyperplasia eventually develop extraophthalmic foci of systemic lymphoma, so patients with this type of conjunctival lesion are usually treated with relatively low-dose external beam radiation therapy to the affected eye or eyes and monitored periodically over the ensuing years for signs of systemic lymphoma. Invasive clinical and pathologic features are generally evident, and regional and distant metastases are potential sequelae. Squamous Cell Carcinoma of Conjunctiva Squamous cell carcinoma of the conjunctiva is an acquired malignant neoplasm composed of very atypical neoplastic cells arising from the stratified squamous epithelium. It usually affects middle-aged to elderly persons except in xeroderma pigmentosum, in which it tends to develop early in life. If neglected or particularly aggressive, conjunctival squamous cell carcinoma can invade the sclera and extend intraocularly, or invade the orbit. At the time of surgery, cryotherapy to the conjunctiva surrounding and the sclera 260 underlying the excisional defect is frequently performed to reduce the chance of local tumor recurrence. If clinical examination or pathologic study of the excised specimen suggests residual neoplasia, supplemental topical therapy with mitomycin C, 5-fluorouracil, or interferon alpha-2b is frequently provided. Conjunctival Melanoma Conjunctival melanoma is an acquired malignant neoplasm that arises from the intraepithelial melanocytes of the conjunctiva. It is almost exclusively unilateral but may be multifocal in some patients if it arises from pre-existing primary acquired melanosis (see earlier in this chapter). It affects both males and females with equal frequency, but it is much more common in Caucasians than in non-Caucasians. It can arise from any region of the conjunctiva (limbal, bulbar, forniceal, palpebral, or caruncular), but it is most common near the limbus in the interpalpebral area. It has the capability of invading the conjunctival stromal, extending into the conjunctival lymphatics, and metastasizing to regional lymph nodes in the head and neck and then to distant organs. Unfavorable prognostic factors for metastasis and metastatic death include larger tumor size, involvement of the fornix or caruncle, and local recurrence following attempted surgical excision. Treatment usually consists of wide surgical excision followed by conjunctival closure using sliding or 262 transposition flaps or a mucous membrane graft.
